Population Health Academy

The Population Health Academy is an exploration of population health, how it’s being applied in the marketplace and all the ways it’s driving health care from a volume-based industry to a value-based industry. This academy is designed for physicians who want data that leads to real, actionable change in their work – and for those physicians, in all types of practices, who believe in a “no outcome, no income” vision for the future of health care. This course provides the opportunity for participants to focus on developing actionable strategies and practical solutions that improve clinical and financial outcomes for their organization.

Through didactic presentations, and group discussions and activities, participants will discuss the key components of a successful population health management strategy.

This program is also designed to provide healthcare and clinical leaders already involved in population health initiatives with the skills to develop and implement models of care that will improve health outcomes.
